Consult Services

Patient consults provide a time for you and your integrative medicine physician to determine the nature of your current medical problems and identify your health goals and expectations. Your first appointment will be 60-90 minutes in length.

During your follow-up visit, you’ll receive an in-depth assessment of your current health situation. The follow-up is usually scheduled about a month or more out in order to make sure all requested diagnostic tests are completed and the information received in our office.

New Consult Patients

If you have an appointment with Dr. Drisko or Dr. Sullivan for a consult, please take the time to read and complete the following forms before your appointment. If you have not completed the required forms prior to your appointment, you will need to reschedule your appointment. NOTE: If you have an appointment with the dietician, please visit this page for necessary forms.

1. Open, print and complete the following forms:
   • Consent for Medical Care
   • New Patient Questionnaire
   • Medical Symptoms Questionnaire
   • Males Only: Testosterone and Health Changes Questionnaire
   • Financial Agreement
2. Open, print and complete a diet diary for at least 2 weekdays and 2 weekend days
